I am wondering what the safety issue is?

Surely if the pintle hook ends upside down (and the ring is upside down) the weight of the trailer ring is being taken by the tongue of the pintle rather than the hook of the pintle and the tongue is being held in place by the locking arrangement not the casting if the hook.  Probably still strong enough for most of the trailers in normal towing but may not be strong enough in extreme conditions.

Having only one of either the ring or the pintle turning means that the pintle can never end upside town - unless the trailer ends upside down  :(.

Here is a photo of the Crump and Cornish trailer without a anti swivel plate. It tends to settle in this position.
The problem occurs when it does settle in this position. The lunette and pintle arrangement is designed to bear load when the lunette is in the horizontal position. As you can see, the load is pushing down on the lunette, effectively pushing it sideways in the pintle. As the trailer and back of the vehicle move up and down, the lunette grabs in the pintle.

You can hear a banging from the rear as the lunette and pintle lock together, release, lock and release.
This picture shows the lunette wearing unevenly. See the notch beginning to wear at about the 11 o'clock position in this photo.

That is why I recommend you don't run with both lunette and pintle unlocked.
